What to bring to your appointment
All patients will be asked for a identification (ID card, driver's license or other form of identification) and an insurance card. Payment is required at the time of service. We accept cash, checks, Visa/Mastercard and Discover. We do not accept "Debit Only" cards.Please bring your medication bottles with you to your appointment.Other Considerations
If you have you completed your new-patient profile and health history, in MyChart, plan to arrive 15 minutes earlier than your scheduled appointment. Otherwise, please arrive 30 minutes prior to your appointment in order to complete your new-patient information.It is also helpful to bring a list of questions for your provider or other health care professional. Many times, patients feel rushed or forget to ask questions when they are in our office.
